WHAT YOU WILL NEED: Have these ready, or apply anyway and we will tell you exactly what is missing. Your WA UBI number, proof of your WA contractor registration or trade license (if your trade requires one), proof of general liability insurance, and a completed W-9. After we review your application, we will send you our short vendor agreement.
